- Classification drift — the drawing says Zone 2, the process changed in 2019, and nobody updated the area classification.
- Dossiers with gaps — equipment installed correctly but the verification record never completed, which is what an auditor actually asks for.
- Mixed-standard equipment — imported plant arriving with certification that does not translate cleanly to Australian requirements.
- Inspection intervals slip — periodic inspection is a schedule, and schedules are the first thing to go when the plant is busy.
